Meridian Health — patient portal
Meridian operates twelve clinics. Their patient portal was a decade-old vendor product with a single-digit adoption rate. We designed and shipped a replacement that actual patients use.

The brief.
Replace a vendor portal that patients and staff both avoided. Build on top of a legacy EHR without rebuilding the EHR. Pass accessibility review without a variance.
What we did.
Shadowed patients in three clinics before drawing a single screen. Rebuilt appointment booking, prescription refills, and secure messaging as independently shippable surfaces so staff could roll them out one at a time.
Delivered a design system that covers forty-one components, all WCAG AA, with audit trail built in.
Outcome.
Monthly active patients tripled inside two quarters. Usability rating from a standard SUS test: 4.8 / 5. Accessibility review passed on first submission.
